SATA Questions: How to Stop Guessing and Start Scoring Points
SATA (select all that apply) questions feel harder than regular multiple choice for one reason: any combination of options can be correct, so you cannot eliminate your way down to a single answer. The fix is the true/false method — judge every option on its own against the question stem, as if it were its own mini true/false question. This guide walks through that method, explains exactly how NGN partial-credit scoring works (+1 per correct selection, -1 per incorrect selection, never below zero), busts the myths that cost points, and solves three full SATA examples step by step.
Why SATA Questions Feel So Hard
A standard multiple-choice question has 4 options and 1 right answer — 4 possible responses. A SATA question with 5 options has 31 possible answer combinations, and a 6-option SATA has 63. You cannot cross off three options and pick what is left, because the correct answer might be one option, three options, or all of them. That is the whole trick: SATA removes the elimination strategy most students lean on, and it punishes pattern-guessing ("there are usually two or three answers") because there is no pattern.
The good news: since the Next Generation NCLEX launched in April 2023, SATA items use partial-credit scoring. You no longer need a perfect selection to earn anything. Get most of the options right and you still bank most of the points — which changes how you should approach every SATA item.
The True/False Method: Judge Each Option Alone
The only reliable SATA strategy is to convert one big question into five or six small ones. Read the stem carefully, lock in exactly what it is asking (expected findings? appropriate interventions? signs of a complication?), then evaluate each option independently — never in comparison to the other options.
- Step 1: Read the stem twice and restate it in your own words. "What findings go with a potassium of 2.9?" is a different question than "what should the nurse do about it?"
- Step 2: Cover the other options and read option 1 alone. Ask: "Is this statement true for this stem — yes or no?"
- Step 3: Mark it T or F and give yourself a one-line reason. If you cannot state a reason, you are guessing.
- Step 4: Repeat for every option, then select every T. Do not go back and "balance" your answer by adding or removing options to hit a certain count.
How SATA Scoring Works Under NGN: Partial Credit
NGN scores SATA items with +/- scoring: you earn +1 for every correct option you select and lose 1 for every incorrect option you select, with a floor of 0 — an item can never score negative. Options you leave unselected earn nothing either way. So a wrong selection actively costs you a point, while a missed correct option only costs you the point you never earned. Here is a worked example on a 6-option item where the key is A, C, and E (3 points possible):

- "There is always more than one correct answer." False. A SATA item can have anywhere from one correct option to all of them. If your true/false analysis says only one option is true, select one and move on.
- "Getting lots of SATA questions means you're passing." The item types you see do not reliably signal your ability estimate. Ignore the meta-game and answer the question in front of you.
- "When in doubt, select it." Under +/- scoring, every wrong selection subtracts a point. Doubt is a reason to leave it out unless you can articulate why it is true.

Run each option through the true/false filter against "low potassium":
- 1. Muscle weakness — TRUE. Potassium drives muscle cell depolarization; low levels cause generalized weakness.
- 2. Flattened T waves — TRUE. A classic ECG change of hypokalemia (along with U waves). Know your NCLEX lab values cold — normal potassium is 3.5–5.0 mEq/L.
- 3. Hyperactive bowel sounds — FALSE. Hypokalemia slows smooth muscle: expect hypoactive bowel sounds and constipation. Hyperactive sounds point the other way.
- 4. Leg cramps — TRUE. Cramping accompanies the muscle irritability and weakness of potassium imbalance.
- 5. Kussmaul respirations — FALSE. Deep, rapid Kussmaul breathing belongs with metabolic acidosis (think DKA), not hypokalemia.

The stem's rule: AP can do routine, standardized tasks for stable clients — never assessment, teaching, or evaluation.
- 1. Vital signs, stable client — TRUE. Routine measurement on a stable client is squarely within AP scope.
- 2. Assist with ambulation — TRUE. Assisting a stable client to walk is a standard AP task.
- 3. Evaluate response to pain medication — FALSE. Evaluation is a nursing-process step; it stays with the RN.
- 4. Teach incentive spirometry — FALSE. Teaching always stays with the RN. AP can remind or reinforce, not teach.
- 5. Measure and record urine output — TRUE. Measuring I&O is routine data collection within AP scope.

Select all that apply: (1) pad the side rails, (2) keep suction equipment at the bedside, (3) place a padded tongue blade at the head of the bed, (4) keep oxygen available, (5) apply restraints during a seizure, (6) keep the bed in the lowest position.
- 1. Pad the side rails — TRUE. Padding protects the client from injury during tonic-clonic movement.
- 2. Suction at bedside — TRUE. Secretions and vomit are airway threats during and after a seizure.
- 3. Padded tongue blade — FALSE. Never put anything in a seizing client's mouth; tongue blades cause airway obstruction and dental injury. Old practice, current distractor.
- 4. Oxygen available — TRUE. Hypoxia is a real risk during a seizure; O2 setup belongs in the room.
